How realrating.care sources Medicare records, documents calculated scores, and handles correction requests.

realrating.care publishes factual summaries of official U.S. government nursing home records plus transparent, reproducible calculations. We describe what Medicare files contain, rank those facts for comparison, and link every facility page to its source profile.

Sources: Primary sources are Medicare open datasets: provider profiles, health and fire deficiencies, resident quality measures, nurse staffing files (where loaded), and skilled nursing facility cost reports. Each facility page links to the corresponding official Medicare nursing home profile so you can verify any figure against the government record.

Calculated fields: Percentiles, composite risk rank, trust-gap flags, and watch-list membership are derived metrics documented in our methodology — transparent formulas applied to public CMS fields.

Language standards: We report citations, ratings, and percentiles numerically and link to the source record. Watch lists filter homes by precomputed metrics so you can compare patterns across a region or operator.

Corrections: If Medicare updates a record or our pipeline mis-parsed a field, tell us through the contact page. We verify against the official Medicare export and republish on the next pipeline run. Include the facility name, the realrating.care page URL, and a link to the official profile when reporting a mismatch.

Advertising: Display advertising may appear on the site. Ad placements never influence scores, sort order, or watch-list membership. Rankings are editorially independent.
