Why watch lists exist
Star ratings summarize a lot — but families often want to know about specific warning patterns before they show up in a single number.
Each list flags a different kind of concern from official Medicare records: worsening resident outcomes, citations that keep repeating, or unreliable staffing reports. A home can have an average star rating and still appear on one of these lists.
The four lists
- Full deteriorationFacilities where every tracked MDS outcome metric worsened Q1→Q4
- Clinical deteriorationFacilities where 4–5 of 5 key outcome measures worsened year-over-year
- Repeat violatorsFacilities with repeat deficiency patterns far above national median
- Staffing integrityCMS citations for falsified or unreliable staffing data
